Kenosha Community Encouraged to Plan for the Future on National Healthcare Decisions Day
In recognition of National Healthcare Decisions Day, The Kenosha Advance Care Planning Community Collaborative is hosting an important community program designed to encourage residents to complete their Advance Directives.
The event will take place on Thursday, April 16, from 12:30 p.m. – 3:00 p.m. at the Froedtert South Kenosha Clinic, ABC Conference Room, 6308 8th Ave, Kenosha.
Advance Care Planning is a vital process that helps adults, regardless of age or current health status, understand and share their personal values and preferences for future medical care. Organizers emphasize that completing these documents is “a gift you give your loved ones to help them make choices about your care.”
The program will feature personal testimonies from community members about what completing an Advance Directive has meant to them. Following the testimonials, attendees will have the opportunity to walk through the Health Care Power of Attorney document with guidance. Witnesses will be available on-site for those ready to complete and finalize their documents.
Event Schedule:
* 12:30 p.m.: Light Lunch (RSVP required)
* 1:00 p.m.: Program Begins
The event is open to any and all community members over the age of 18.
RSVP is required for the light lunch. To reserve your spot, please call (262) 652-4482 x 1268.
