Learn More About Advance Care Planning
In recognition of National Healthcare Decisions Day, the Kenosha Advance Care Planning Community Collaborative invites community members to a special event focused on encouraging the completion of Advance Directives. Attendees will hear personal stories from local residents about the impact of completing their Advance Directive and will have the opportunity to walk through the Health Care Power of Attorney document. Witnesses will be available on-site for those ready to complete their forms.
